Qamar Raza, M.D. – Executive Medical Director

Dr. Qamar Raza is executive medical director and has extensive experience working in primary care/internal medicine and emergency room settings. Before moving to his current position, Dr. Raza served as medical director and assistant professor at the Department of Internal Medicine at Texas Tech University of Health Sciences in Midland, TX, and emergency room physician at Midland Memorial Hospital. In addition, Dr. Raza previously served as adjunct faculty at Ohio State University and University of Findlay in Ohio. He earned his MBA from Tiffin University and is certified by the American Board of Internal Medicine.

David Hall, M.D. – Medical Director

Dr. David Hall serves as medical director and has more than 20 years of experience working in primary care settings, urgent care/emergency rooms, medical student teaching and administrative roles. Previously, Dr. Hall served as an assistant professor at the Department of Family Medicine in the McGovern School of Medicine (part of the University of Texas Health Science Center in Houston) and was medical director at the El Franco Le Health Center. Dr. Hall is a native Texan who is board certified in family medicine. He attended medical school at the University of Texas Medical Branch in Galveston and completed a residency in family medicine at Baylor College of Medicine in Houston.

‘Seun Akeredolu, P.A. – Physician Assistant

‘Seun Akeredolu is a board-certified physician assistant with experience caring for patients of all ages. Akeredolu graduated from Baylor College of Medicine in 2010. She earned her undergraduate degree in Biomedical Sciences at Texas A&M. She is qualified to treat a wide range of medical conditions in addition to performing general physicals, gynecological exams and well child exams.

Mansi Shah – Physical Therapist

Mansi is a dedicated and compassionate physical therapist with over 8 years of experience. She earned her Bachelor of Physical Therapy degree from Rajiv Gandhi University in India in 2008 and moved to the USA in 2015. Currently, Mansi is pursuing her Doctorate of Physical Therapy at the University of Montana, which she expects to complete in April 2025. Her pursuit of advanced education reflects her commitment to staying at the forefront of the field and offering her patients the highest standard of care.

Mansi is certified in Dry Needling and the McKenzie Method for the Lumbar Spine, enabling her to provide specialized treatments that reduce pain and restore function. Her patient-centered approach ensures that every individual receives a personalized care plan tailored to their unique needs and goals.

Mansi is passionate about helping her patients recover and thrive, empowering them to live active and fulfilling lives.

Outside of her professional life, Mansi is a proud mother of young twins and enjoys spending quality time with her supportive husband. She loves cooking, socializing, and creating a warm, welcoming environment in her home and workplace.

Douglas Yauch, DDS – Dentist

Dr. Douglas Yauch grew up in Michigan and received his Dental degree from the School of Dentistry, Medical College of Virginia, located in Richmond. He and his wife moved to Texas to be closer to family and enjoy the mild winter season. He believes in quality care for his patients and strives to provide a positive environment that promotes a relaxed and caring atmosphere.

Kevin Urada, DDS

Dr. Kevin Urada grew-up in Honolulu, Hawaii. He obtained his Doctor Dental Surgery degree from the University of Washington School of Dentistry. He relocated to Houston, Texas to provide a better education for his children and for the job opportunities this city provided. He believes in quality dental care and strives to make the patient feel as comfortable as possible. He hopes to make friends, rather than patients. His hobbies include fishing, camping, and has a great love for the game of baseball.

Dolores Mora, DDS

Dr. Mora was born in Santa Ana California, raised by loving grandparents in Freeport, Texas from the age of five. Married to a supportive husband and blessed with three beautiful daughters.  Received a BA from PV A&M in Biology and Chemistry. Doctor of Dental Surgery was received at U.T. Dental Branch in Houston School of Dentistry. Residency in Monterey where she received her specialty in Orthodontics. In 1996 Dr. Mora was a sole proprietor of a Dental Office until 2022.  

Dr. Mora also taught at U.T. Dental School where she was able to share her passion for dentistry. Due to the symbiotic relationship, she was able to give the best care to her patients. To relax and release stress Dr. Mora loves fishing with her husband and playing pickleball.  

Michelle Wong, OD – Optometrist

Dr. Michelle Wong is board certified by the National Board of Examiners in Optometry to practice therapeutic optometry. She is licensed as an optometric glaucoma specialist by the Texas Optometry Board. Dr. Wong’s professional memberships are with the American Optometric Association and the Texas Optometric Association. Dr. Wong received her B.S. in Biology from the University of Texas at Austin and her O.D. from the Illinois College of Optometry in Chicago.

Nancy Nghe, OD – Optometrist

Dr. Nancy Nghe is a licensed therapeutic optometrist and glaucoma specialist. Dr. Nghe received her Bachelor of Science in Medical Laboratory Science at the University of Rhode Island. She obtained her Doctor of Optometry degree at the State University of New York College of Optometry. During her time at SUNY College of Optometry, Dr. Nghe completed her clinical training at Westpoint's Keller Army Community Hospital, VA Boston Health Care System, and Woodhull Medical and Mental Health Hospital. Dr. Nghe has expertise in managing a variety of ocular diseases, pediatrics, specialty contacts in addition to providing primary eye care.

Outside the office, she has devoted her time volunteering for non-profit organizations, Dr. Nghe enjoys working out and traveling.

Punam Patel, Pharmacist

Punam is originally from Memphis, TN, which is where she received her Doctor of Pharmacy from University of Tennessee Health and Science Center. She is a recent Houstonian and is enjoying her time exploring our beautiful city! Punam loves spending time with her family and friends, baking, reading novels, and running.

Jan Milligan – Pharmacist

After receiving her Bachelor of Science in Pharmacy Degree from Purdue University, Jan Milligan, R.Ph., moved to Houston. She and her husband, Mark, are empty nesters since their son, Rob, graduated from Texas Tech and married. Jan enjoys getting to know BP employees and their families and being an integral member of the Westlake Health & Wellness Center team.
